Transitional Aged Care Packages
After a stay in hospital, usually the place you want to go is to your own home. But if you need support and assistance in order to recover, a Transitional Aged Care Package (TACP) might be what the doctor ordered.
To received Transitional Care you first need to be assessed and approved as eligible by an Aged Care Assessment Team (ACAT) while you are still in hospital. A member of the ACAT (a doctor, nurse or other health care professional) will visit you and talk about how you're coping and they will work out if you would benefit from Transitional Care.
Once you have been approved and you understand what support you're eligible for, the ACAT then contacts us to tell us what support we can provide you.
The support we provide is for a limited time, just to assist you until you have recovered from your hospital stay.
